Superficial radiation therapy (SRT) for nonmelanoma skin cancers is payable only in non-facility settings and requires documentation that SRT is medically necessary over other modalities, including a provider statement that the patient is a nonsurgical candidate and evidence of informed discussion. Comprehensive, legible medical record documentation is required (diagnosis, history, imaging, treatment plan, dosimetry, DVH review, signatures of responsible clinicians and physicist, CTV/PTV verification and patient‑specific QA), and billing must use CPT/HCPCS consistent with ASTRO/CMS guidance; non-covered services must be billed with appropriate modifiers (GY/GZ).
