This billing/coding guidance states that superficial radiation therapy (SRT) for nonmelanoma skin cancers (NMSC) is payable only in non-facility settings and must meet medical necessity as documented in the medical record. Coverage requires documentation that the patient is a nonsurgical candidate or has a documented preference for SRT after discussion of risks, and detailed radiation oncology planning, dosimetric verification, and signed physician/physicist attestations; correct CPT/HCPCS and ICD-10-CM coding and use of modifiers (GY/GZ for non-covered Part B services) are required.
