Policy Summary
SRT for nonmelanoma skin cancers is covered only when medically necessary per LCD DL40193 and is payable exclusively in non-facility (freestanding) settings. Coverage requires pre-treatment documentation that the patient is a nonsurgical candidate with informed discussion of risks and a comprehensive medical record detailing treatment planning, dosimetry, target verification, signatures/credentials, and coding specificity; records must be legible and available to the contractor upon request.
