This article provides billing and coding instructions for reporting Non-Next Generation Sequencing targeted molecular panel tests for predictive cancer testing. Claims must include the appropriate CPT code, exactly 1 unit of service, the appropriate DEX Z-Code placed adjacent to the CPT in the specified claim fields for Part A or Part B and claim format, and an appropriate ICD-10-CM diagnosis code. Do not add extra characters to the DEX Z-Code when entering it on SV101-7 for Part B electronic claims.
