Policy Summary
CPT 95165 covers the physician preparation and provision of antigen extracts for subcutaneous allergen immunotherapy when prepared by an MD/DO who has examined the patient and established a treatment plan; administration/injection is billed separately. Billable doses should reflect the number of 1 cc aliquots represented by the vial volume (standard 10 cc vial = maximum 10 doses), supplies should not exceed a 12-month supply, and maintenance claims require modifier EJ; same-day visits with injection codes require documentation of a separately identifiable service.
