This policy provides billing and claim submission instructions for Non-Next Generation Sequencing targeted molecular panel tests used for predictive testing in cancer. Claims must include the appropriate CPT code with 1 unit of service, the matching DEX Z-Code entered adjacent to the CPT code in the specified comment/narrative fields for Part A and Part B (using the listed electronic or paper form fields), and the appropriate ICD-10-CM diagnosis code; do not add extra characters to the DEX Z-Code field SV101-7.
