Aetna covers a comprehensive history/physical and psychosocial evaluation, duplex (Doppler/ultrasound) with intracorporeal papaverine, pharmacologic response testing (e.g., papaverine, phentolamine, PGE1), and DICC or pudendal arteriography only when performed for members who meet medical-necessity criteria and are being considered for penile re-vascularization. Routine nocturnal penile tumescence testing is not routinely necessary (RigiScan only if simpler snap/gauge tests are equivocal), coverage of injectable ED drugs depends on the member’s benefit plan, and exogenous testosterone and topical vasodilator creams are experimental/investigational for non‑hypogonadal impotence.
