Aetna covers injectable medications when no appropriate oral alternative exists—including self‑administered drugs and established off‑label uses—and excludes experimental/investigational drugs, drugs for non‑covered services, infertility injectables only unless required by regulation or benefit design, and steroids for performance enhancement. Coverage requires the medication be medically necessary and appropriate; policy trial requirements may be waived for documented needle phobia meeting DSM‑5 criteria with attempted management and counseling (ICD‑10 F40.231 when criteria are met).
