Aetna covers automated ambulatory blood pressure monitoring to confirm hypertension and distinguish white‑coat hypertension (adults require ≥2 office readings on separate days with systolic ≥140 and/or diastolic ≥90 mm Hg; children >90th percentile), for episodic hypertension suspicious for pheochromocytoma, evaluation of hypotensive symptoms/medication‑related syncope, and for syncope when used with a 24‑hour Holter. Key requirements/exclusions: monitoring must be 24 hours up to 3 days (not <24 hrs or >3 days), repeat testing is generally no sooner than every 6 months, it is considered experimental/investigational for heart failure and for pregnant women who lack listed indications, and it is not covered for diagnosing malignant hypertension.
