Policy Summary
Aetna covers vasectomy reversal (CPT 55400) and epididymectomy (CPT 54860/54861) for treatment of post‑vasectomy pain syndrome (ICD‑10 G89.28) only when selection criteria are met and the member has failed NSAIDs and local nerve blocks/steroid injections. Endoscopic vasectomy and device‑based occlusion methods (e.g., Vasclip, Pro‑Vas, RISUG, Intra Vas Plug, LigaSure) and certain procedures for PVPS such as micro‑denervation of the spermatic cord are considered experimental/investigational and are not covered (specific CPTs for some devices are not listed).
