Aetna covers ACIP-recommended standard or preservative-free trivalent/quadrivalent injectable influenza vaccines for routine annual use in persons aged ≥6 months and covers intranasal live-attenuated influenza vaccine (LAIV4) only for ages 2–49; CPT codes 90660 and 90664 and LAIV use outside the 2–49 age range are not covered. Coverage requires adherence to ACIP guidance and excludes vaccination for persons with specified contraindications — e.g., severe allergy to vaccine components (including egg), pregnancy/nursing (for LAIV), chronic heart or lung disease including asthma, immunocompromise or immunosuppressive therapy, recent wheeze in young children, aspirin use in 2–17-year-olds, and prior Guillain–Barré syndrome within 6 weeks unless benefits outweigh risks.
