Hepatitis A Vaccine

Aetna covers hepatitis A vaccine for routine immunization in persons ≥12 months (including children 12–23 months and catch‑up for ages 2–18), for certain high‑risk groups (HCT recipients post‑transplant, homeless persons ≥1 year, people with HIV) and considers it medically necessary for post‑exposure prophylaxis (PEP) when initiated within 2 weeks of exposure. Limitations: vaccine is not approved for routine use <12 months (a 6–11 month travel dose does not count toward the 2‑dose series), Twinrix is for adults ≥18, many Aetna HMOs exclude travel/work immunizations, ACIP prefers immune globulin (IG) over vaccine for PEP in children <12 months, immunocompromised persons, those with chronic liver disease, and adults >40 (and immunocompromised/chronic liver disease persons exposed within 14 days who haven’t completed the series should receive both IG [0.1 mL/kg] and vaccine simultaneously at different sites).
