Stereotactic Radiosurgery

Aetna covers cranial stereotactic radiosurgery (CyberKnife, Gamma Knife, LINAC) for symptomatic small (<3 cm) AVMs, aneurysms, and specified benign tumors when unresectable or the patient cannot tolerate surgery; for trigeminal neuralgia refractory to conservative treatments; for severe essential tremor or disabling Parkinsonian tremor when medical therapy has failed and thalamotomy criteria are met but surgery is not an option; and for brain malignancies in patients with KPS 80–100, controlled systemic disease, and ≤4 metastases (additional lesions may be considered with justification). All other sites/indications are considered experimental/investigational and not covered (including cluster headaches, epilepsy except when related to AVM/tumor treatment, and mammographic microcalcifications).
