Aetna covers fetal echocardiography (including Doppler and color flow mapping) after 12 weeks’ gestation only for specified indications: maternal type 1 diabetes or pregestational type 2 on insulin in the first trimester, systemic lupus erythematosus, a first‑degree relative with congenital heart disease, fetal nuchal translucency ≥3.5 mm, or following an abnormal/incomplete anatomic (4‑chamber) cardiac scan. It is not medically necessary if the 4‑chamber view is adequate with no other concerns, and all other uses (e.g., advanced maternal age screening, gestational diabetes including insulin after the first trimester, modafinil exposure) are considered experimental/investigational and not covered.
