Chiropractic services are covered only for members with a documented neuromusculoskeletal disorder when medical necessity is clearly documented and objective improvement is shown within the initial 2 weeks (home-based care allowed if homebound); if no improvement at 2 weeks further treatment is not medically necessary unless modified, and no improvement within 30 days despite modification or after maximum therapeutic benefit is reached means continued care is not covered. Excluded are manipulation for asymptomatic persons, maintenance/preventive/supportive care, care for conditions that are neither improving nor regressing, and treatment of idiopathic scoliosis beyond early adolescence unless pain/spasm or other medical indications exist.
