Aetna covers medically necessary prosthetic external lenses and intraocular lenses for aphakia/post‑cataract surgery and will replace pediatric eyeglasses under a pediatric vision benefit when there is a refractive change, significant breakage/marring that impairs vision or safety, or a medically necessary frame change (growth, metal allergy, etc.). Many Aetna medical plans otherwise exclude routine eyeglasses/contacts (including replacements for loss, damage, or solely refractive changes), Medicare/HMO members are generally limited to one post‑cataract pair (and some HMOs only cover an initial prosthetic), so coverage and replacement eligibility depend on the specific plan.
