Policy Summary
Aetna considers home uterine activity monitoring (HUAM) experimental and investigational and does not routinely cover it (CPT 99500 excluded) except by individual case review. Coverage may be allowed only for women >18 weeks who cannot feel contractions and have specific complications—documented cervical incompetence (cerclage or funneling), placenta previa with hemorrhage, or history of classical cesarean/deep myomectomy with contractions/cervical change—and is not considered medically necessary for twin gestations without extenuating circumstances.

Key requirements from the full policy
"Use of HUAM is associated with increased unscheduled antenatal visits and more prophylactic tocolytic drug therapy (per Cochrane review), and no consistent impact on maternal or perinatal mortality..."
