Fetal Fibronectin, Inflammatory Biomarkers, and Salivary Hormone Testing for Preterm Labor

Fetal fibronectin immunoassay (CPT 82731) is covered for symptomatic pregnant women at risk for preterm delivery (and may be repeated if symptoms persist ≥2 weeks after a prior negative), whereas routine screening, testing asymptomatic high‑risk women, use to select or follow cervical cerclage, salivary hormone testing (HCPCS S3652), and intra‑uterine inflammation biomarkers (e.g., cytokines, MMP‑9) are not covered. Coverage requires ACOG selection criteria: intact membranes, cervical dilation <3 cm, sampling between 24w0d and 34w6d gestation, with specified ICD‑10 codes when criteria are met.
