Aetna covers INR home testing devices for patients on chronic warfarin therapy for mechanical heart valves, ventricular assist devices, chronic atrial fibrillation, deep venous thrombosis, or pulmonary embolism, but deems the devices experimental/investigational for other indications and considers additional hardware/software for data downloading not medically necessary. Coverage requires an expected need of ≥6 months, at least 3 months of prior anticoagulation before home use, and self-testing no more than once weekly.
