Aetna covers intra‑articular hyaluronate viscosupplementation for knee osteoarthritis only when there is radiographic evidence of OA (e.g., joint‑space narrowing, osteophytes, subchondral changes) or the member meets the clinical criteria (at least five specified signs/symptoms). All other uses are considered experimental/investigational — including intra‑articular polynucleotides, ultrasound/fluoroscopic/arthrographic guidance, combinations with anesthetics/corticosteroids/mannitol/sorbitol/mesenchymal stem cells/PRP (local anesthetic to numb the injection site is allowed), and amobarbital/hyaluronic acid hydrogel for post‑traumatic OA prevention.
