Covered: panniculectomy/apronectomy when the pannus hangs below the distal symphysis pubis and causes chronic intertrigo (documented with high-quality frontal/side photos including lifted pannus), repair of true incisional/ventral hernia, liposuction/suction lipectomy for lipedema (and trunk lipectomy for trunk symptoms) after ≥3 months of failed conservative therapy and meeting diagnostic criteria, and lipectomy for lymphedema when selection criteria are met. Excluded/limited: panniculectomy to prevent hernia, repair of diastasis recti, abdominoplasty/lipoabdominoplasty and suction lipectomy for cosmetic indications, and several listed procedures are considered experimental/investigational.
