Covered: iontophoresis is covered for topical anesthetic delivery before emergent skin puncture/dermatologic procedures, for intractable primary focal hyperhidrosis (axillae, palms, or soles) only when the member is unresponsive/intolerant to oral therapy, has significant social/professional disruption, and topical antiperspirants have failed or caused severe rash, and for fentanyl iontophoretic transdermal system for acute postoperative pain and pilocarpine sweat testing for cystic fibrosis. Excluded/limited: iontophoretic administration of acetic acid (rotator cuff disease, heel pain/bone spur), acetic acid plus corticosteroid (epicondylitis), acetylcholine or sodium nitroprusside (cardiovascular risk assessment), ascorbic acid for hyperpigmentation, and other unproven indications are considered experimental/investigational.
