Aetna covers combined steroid/anesthetic injection (CPT 20550) for plantar fasciitis after failure of conservative measures (stretching, OTC silicone heel inserts, and 2–3 weeks of NSAIDs) and endoscopic plantar fasciotomy (CPT 29893) as an alternative to open surgery after a failed 6‑month trial of conservative therapy (coverage requires meeting the policy selection criteria; ICD‑10: M72.2, M77.30‑M77.32). Numerous other treatments — e.g., PRP/autologous blood/growth factor injections, ESWT devices, gastrocnemius recession, Tenex/TenJet, botulinum toxin, many listed CPT/HCPCS/J‑codes — are considered experimental/investigational or not covered, and heel cushions/orthotics are not covered under plans that exclude such devices.
