Aetna covers caloric vestibular testing, dynamic (head‑shaking) visual acuity, head impulse/head‑thrust, optokinetic and spontaneous nystagmus testing for diagnosis/evaluation of chronic vertigo or Ménière’s disease, and considers ENG/VNG investigational when used for indications other than vestibular disorders (dizziness/vertigo/imbalance). Epley and Semont maneuvers are covered only if both a positive Hallpike and ≥4 months of symptoms are present (and are investigational for patients with CNS disorders such as temporal lobe epilepsy, MS, cerebrovascular disease, vertiginous migraine, CPA tumors, or cerebellar lesions); vestibular rehabilitation is limited to chronic vertigo/PPPD meeting criteria and is not continued beyond 12 visits without documented improvement.
