Speech therapy is covered for communication and swallowing disorders (including home/home‑health or hospice post‑hospital care, auditory rehabilitation for cochlear implants/hearing aids, and idiopathic speech delay in children ≥18 months) but is not covered when duplicative of OT/PT, solely maintenance, routine/non‑skilled care, or for self‑correcting developmental dysfunctions. Coverage requires physician documentation that significant improvement is expected within a reasonable/predictable period, services by a licensed/certified SLP (or supervised ancillary within scope), and an ongoing written plan of care reviewed/approved by the treating physician (with homebound or implant selection criteria met as applicable).
