External Counterpulsation (ECP)

External counterpulsation (HCPCS G0166) is covered for up to 35 sessions for disabling chronic stable angina (NYHA Class III–IV) that is refractory to maximum medical therapy and not amenable to revascularization (inoperable, high surgical risk, unfavorable coronary anatomy, or prohibitive comorbidity); repeat courses are allowed only if prior ECP produced sustained benefit (≥25% reduction in angina frequency or ≥1 angina class improvement) and ≥3 months have elapsed. Hydraulic ECP devices and extension beyond 35 sessions are not medically necessary, and ECP is considered experimental/not covered for numerous other indications (e.g., unstable angina, heart failure, peripheral vascular disease, erectile dysfunction, stroke, Long COVID, etc.).
