Ablation of Hepatic Lesions

Aetna covers cryo-, microwave-, or radiofrequency ablation only for isolated colorectal liver metastases or isolated hepatocellular carcinoma in patients who are not candidates for open surgical resection; ablation is experimental/investigational when criteria are not met, for metastases from non-colonic primaries, for palliative use, and electro‑chemotherapy and histotripsy are excluded. Key requirements: disease must be confined to the liver with all tumors deemed destroyable on preoperative imaging, no nodal or extrahepatic metastases, and the patient must be unsuitable for open resection due to tumor location/extent or comorbidities.
