Policy Summary
Aetna covers stereotactic cingulotomy (CPT 61720, 61735) only as a medically necessary, last‑resort treatment for terminal cancer–related pain (ICD‑10 G89.3, Z51.5) when documented selection criteria are met; anterior (ventral) capsulotomy and stereotactic cingulotomy for chronic non‑malignant pain, drug addiction, intractable seizures, and psychiatric disorders (e.g., OCD, mood, anxiety, personality, schizophrenia, Tourette’s, eating disorders) are considered experimental/investigational and not covered.
