Aetna covers transcatheter closure of ASDs (including fenestration closure after a fenestrated Fontan and secundum ASDs), PDA closure using the Amplatzer or other FDA‑approved devices, transcatheter VSD closure for complex VSDs in patients at high surgical risk, and PFO closure for adults 18–60 with cryptogenic stroke — all requiring FDA‑approved devices and indication‑specific criteria. Devices or procedures not FDA‑approved for the indication (including non‑approved PDA devices, the APTURE shunt, Neovasc Reducer, Nit‑Occlud Lê VSD coil for peri‑membranous VSD, and percutaneous inter‑atrial septal shunts) are considered experimental/investigational and excluded.
