Corneal Graft and Amniotic Membrane Transplantation, Corneal Stromal Lenticule Transplantation, Limbal Stem Cell Transplantation, or Sural Nerve Grafting for Ocular Indications

Preserved human amniotic membrane transplantation or limbal stem cell transplantation is covered for ocular surface reconstruction in limbal stem cell deficiency refractory to conventional treatment when due to total loss of stem cells (one eye only), chemical/thermal injuries, contact lens–induced keratopathy/toxic effects, or corneal limbal dermoid. Allogeneic corneal epithelial stem cell transplantation and specified uses of amniotic membrane (for age‑related macular degeneration, combined amnion for infectious scleritis, heparinized‑AM with growth factors for alkali burns, and HLA‑matched limbal allograft with AM to prevent graft rejection) are considered experimental/investigational and excluded.
