Aetna covers radiofrequency ablation (open, laparoscopic e.g., Acessa, or transcervical e.g., Sonata) and transcatheter uterine artery embolization as alternatives to hysterectomy or myomectomy for symptomatic uterine fibroids, and allows selected use of power morcellation only in specified low-risk or high-risk surgical candidates; procedures must meet the symptom criteria (persistent fibroid-attributed menorrhagia, bulk-related pain/pressure, urinary symptoms, or dyspareunia) and be billed with the listed CPT/HCPCS codes. Myomectomy or hysterectomy with power morcellation is generally excluded except for the limited exceptions, other uses of UAE and listed treatments (e.g., focused ultrasound, cryotherapy, acupuncture) are experimental/not covered, and only procedures meeting the policy’s selection criteria are reimbursed.
