Behavioral vision therapy (visual perceptual training) and vision restoration therapy (including when combined with tDCS) are considered experimental/investigational and are excluded from coverage for all indications — including visual perceptual dysfunctions and visual field deficits from ischemic optic neuropathy, neurotrauma, or stroke. No indications or CPT/HCPCS codes are covered because effectiveness has not been validated in well‑designed prospective clinical studies.
