Aetna covers preconceptional sperm-sorting to enrich for X-bearing sperm only when medically necessary to prevent conception/birth of a male in a woman known to be heterozygous for a seriously handicapping X‑linked condition (e.g., Lesch‑Nyhan); sex selection for personal or family reasons (including family balancing) is excluded. Assisted reproductive procedures done solely for sex selection (e.g., IVF/ICSI) are not considered treatment of disease and coverage for IVF/ART or post‑sorting infertility services depends on the member’s plan and applicable state mandates, and sperm sorting does not guarantee fetal sex.
