Aetna covers optic nerve and retinal imaging for specific diagnoses — age‑related macular degeneration, cystoid macular edema after cataract surgery, diabetic retinopathy, ethambutol‑induced optic neuropathy, and glaucoma — but optic nerve imaging for glaucoma more than once per year is not medically necessary. Imaging is considered experimental/investigational and not covered for numerous other indications (e.g., routine asymptomatic glaucoma screening, annual thyroid‑ophthalmopathy follow‑up, cataracts, dry eye, retinal biomarkers for neurodegeneration, fingolimod monitoring, and patient‑initiated remote OCT transmission) and certain CPT (0469T; 0604T–0606T) and listed ICD‑10 codes are excluded.
