Aetna covers evaluation of recurrent pregnancy loss (RPL defined as two or more consecutive spontaneous abortions), including endometrial biopsy for luteal phase defect, hysterosalpingography/hysteroscopy/sonohysterography to evaluate uterine anatomy, parental karyotype, and Factor V Leiden genetic testing only when an activated protein C (APC) resistance assay is abnormal. Numerous other genetic, immunologic, biochemical, embryologic and reproductive tests (e.g., MTHFR, PGT‑A, sperm DNA fragmentation, many thrombophilia and cytokine polymorphism tests) are considered experimental/investigational and are not covered for RPL evaluation.
