Pancreaticoduodenectomy (Whipple) is covered for ampullary adenoma/adenocarcinoma, cholangiocarcinoma, chronic pancreatitis, combined pancreatic/duodenal injury, and duodenal neoplasm; fibrin sealant is considered integral to the procedure and is not separately reimbursed. The procedure is considered experimental/investigational (and not covered) for Zollinger‑Ellison syndrome, preoperative biliary drainage and use of an omental flap to prevent postoperative complications are investigational, and CPT 49905 (omental flap, intra‑abdominal) is not covered for these indications.
