Aetna covers Mohs micrographic surgery as medically necessary for lesions in tissue-preservation areas (ears, face, feet, hands, genitalia, perianal), atypical fibroxanthoma, dermatofibrosarcoma protuberans, and exceptionally large (≥2 cm) or rapidly growing lesions, but considers Mohs investigational for all other indications (e.g., deep cutaneous fungal infections), for vulvar malignancies, for use of optical coherence tomography for margin definition, and for skin substitutes for Mohs wounds. Key requirement: a single physician must perform both the surgical and pathological roles (the Mohs CPT codes are inappropriate if either role is delegated and billed separately).
