Aetna covers pars plana vitrectomy for medically necessary indications such as macular hole repair, proliferative retinopathy, rapidly progressing infectious endophthalmitis, retinal detachments secondary to vitreous strands, and when needed to administer voretigene neparvovec for RPE65‑related retinal dystrophy (see CPB 0927). Aetna excludes prophylactic PPV for acute retinal necrosis, PPV for asymptomatic vitreous floaters, combined PPV with phacoemulsification or with scleral buckling for retinal detachment, and does not require intra‑operative intraocular pressure monitoring due to lack of evidence for outcome benefit.
