Aetna covers polio vaccination as medically necessary per ACIP—routine childhood IPV (4 doses at 2 mo, 4 mo, 6–18 mo, and 4–6 yrs) and IPV for adults at increased exposure risk; OPV (oral, live) is restricted to limited circumstances (parental refusal of IPV for the 3rd/4th dose after VAPP counseling, mass outbreak campaigns, or unvaccinated children traveling <4 weeks to endemic areas) and some plans may exclude travel- or work-related immunizations. Key requirements: minimum age 6 weeks, minimum 4-week intervals between doses 1–2 and 2–3, adult IPV primary series of 2 doses 1–2 months apart with a 3rd at 6–12 months, and OPV should not be given to hematopoietic stem cell transplant recipients.
