Aetna covers hepatitis B vaccination for infants (regardless of maternal HBsAg status), children/adolescents <19, adults 19–59 who haven’t completed vaccination, adults ≥60 with or without risk factors, and persons at sexual risk, but excludes boosters for immunocompetent persons, travel/work‑related immunizations, and any other indications (including lymphoma prevention) as investigational. Key requirements/limitations: revaccination and boosters are generally not medically necessary for persons with normal immune status, Aetna considers non‑listed uses experimental, and CDC does not recommend post‑booster anti‑HBs testing 1–2 months later in hemodialysis patients.
