Aetna covers balloon dacryocystoplasty for mucocele of the lacrimal sac, chronic dacryocystitis or conjunctivitis from lacrimal sac obstruction, congenital nasolacrimal duct obstruction not resolved by probing in patients over 1 year of age, epiphora from acquired sac/duct obstruction, and lacrimal sac infection that must be relieved before intraocular surgery. It deems balloon dacryocystoplasty (and silicone stenting with it), osteopathic manipulation, and inferior turbinate fracture experimental/investigational and not covered for all other indications—specifically excluding cases with anatomic/bony canal malformations, dacryocystolithiasis, recurrent active dacryocystitis, sarcoidosis, lacrimal sac tumors, Wegener granulomatosis, post‑traumatic bony canal obstruction, and other unlisted uses.
