Aetna covers carbon dioxide laser for condyloma; palliation of melanoma with in‑transit metastases when palliative surgery is not feasible; pelvic pain from endometriosis; primary penile tumors limited to clinical stage Tis, Ta, and T1 Grade 1–2; and refractory plantar warts. It considers the procedure experimental and investigational (not covered) for aphthous stomatitis, cutaneous angiokeratomas, cutaneous leishmaniasis, elephantiasis nostras verrucosa, and female sexual dysfunction because effectiveness has not been established.
