Transcranial Magnetic Stimulation and Cranial Electrical Stimulation

TMS is covered as medically necessary for adults ≥18 with psychiatrist‑confirmed severe major depressive disorder (single or recurrent, without psychosis) when delivered with an FDA‑cleared device per labeled indications and monitored with baseline and serial standardized depression rating scales. It is not medically necessary/considered experimental for patients with substance abuse in the past 90 days, active suicidality, metal implants in/around the head, or specified neurological conditions (cerebrovascular disease, dementia, severe/repetitive head trauma, increased intracranial pressure, or CNS tumors).
