Aetna covers full- or half-leg intermittent pneumatic compression (IPC) devices as medically necessary DME for chronic venous insufficiency with venous stasis ulcers unhealed after a 6‑month physician‑directed trial of conservative therapy (compression bandage/garment, appropriate dressings, exercise, elevation) and covers lower‑extremity IPC to prevent DVT in patients who are bedridden due to trauma, orthopedic or neurosurgery, or other conditions preventing ambulation; single‑patient systems (e.g., VenaPro), trunk IPC for post‑orthopedic DVT prevention, HCPCS E0675 for the listed indications, and IPC for uses like enhancing Achilles tendon healing are excluded or considered investigational/not medically necessary. When IPC is indicated, non‑segmented or segmented devices without manual per‑chamber pressure control are generally adequate, while segmented devices with manual chamber control require clear, case‑specific documentation of medical necessity, and "bedridden" does not include having a cast/splint, use of an assistive device, or non‑weightbearing status alone.
