Breast Ductal Lavage and Fiberoptic Ductoscopy

Covered when used to diagnose non‑lactational sporadic nipple discharge (ductal lavage as an alternative to failed nipple aspiration and fiberoptic ductoscopy with cytology for intraductal lesions) and when fiberoptic ductoscopy is used intraoperatively to guide resection of known intraductal breast cancer. All other uses—including breast cancer screening, risk assessment, treatment of mastitis, contralateral evaluation with ductal endoscopy plus lavage, and molecular assays of ductal fluid (methylation or miRNA)—are experimental/investigational and not covered; ductoscopy diagnostic use requires documented positive cytology and intraoperative use requires known intraductal cancer.
