Aetna covers transvaginal ultrasonography for evaluation of pelvic masses (e.g., adenomyosis, cysts, fibroids, cancer), bowel endometriosis, ectopic pregnancy, vasa previa, and abnormal uterine bleeding, but considers TV‑US experimental/investigational for screening for endometrial, ovarian, or other gynecologic cancers and views Doppler mode as not medically necessary for TV‑US monitoring in women with Lynch II syndrome. Pelvic (abdominal) echography is integral to TV‑US and not separately reimbursed (and is duplicative when pelvic structures are within the bony pelvis), and CPT 76830 is generally not separately payable except for confirmation of IUD placement after insertion.
