Aetna covers balloon‑expandable venous stents only for specific indications — Budd‑Chiari syndrome, chronic iliac and ilio‑caval occlusions, ilio‑femoral thrombosis from iliac compression (May‑Thurner), and post‑operative venous narrowing after sinus venosus ASD repair — with CPTs 37248/37249 covered only if selection criteria are met. All other uses (e.g., central vein stenosis in hemodialysis patients [Z99.2], porto‑mesenteric/porto‑systemic reconstruction) are experimental/investigational and not covered, and stent‑grafts are considered unsuitable for peripheral fistula stenoses due to crush risk.
