Vitamin B-12 Therapy

Aetna covers intramuscular or subcutaneous vitamin B‑12 injections only for members with current or previously documented B‑12 deficiency associated with fish tapeworm anemia (B70.0), macrocytic/megaloblastic anemia (D53.1) or pernicious anemia (D51.0), and most plans exclude oral vitamins or B‑12 given solely as a nutritional supplement. Initial parenteral therapy is typically 1,000 mcg daily for 5 days then weekly for 4 weeks with maintenance 1,000 mcg every 1–3 months; treatment beyond 2–3 months or more frequent dosing than these usual schedules requires medical review to establish continued medical necessity.
