Endothelial cell photography is covered only for the listed indications—pre-fitting for extended-wear lenses after intraocular surgery, before secondary IOL implantation, prior to procedures with higher corneal endothelial risk, for patients with prior intraocular surgery needing cataract surgery, or with slit-lamp evidence of corneal edema—and is experimental/investigational for all other uses. It’s considered integral (not separately payable) when performed as part of a preoperative cataract exam if cataract is the only visual problem, and CPT codes 92286/92287 are payable only when the policy’s selection criteria are met.
