Immune Globulins for Post-Exposure Prophylaxis

Aetna covers specific immune globulins for defined post‑exposure indications—Hepatitis B Ig for listed HBV exposures (including infants of HBsAg‑positive mothers, percutaneous/permucosal exposures, sexual contacts, household infant exposure <1 year and indefinite prophylaxis in HBsAg‑positive liver transplant recipients), CMV‑Ig for severe CMV disease in transplant recipients and prophylaxis for CMV‑negative renal recipients of CMV‑positive donors, plus listed products (Rho‑D, rabies, VZIG, tetanus, vaccinia, BabyBIG, IGIM/GamaSTAN); all other uses are considered experimental/investigational and not covered. Key requirements/limitations: precertification is required for IGIM (GamaSTAN), IGIM is IM only and must not be used for hepatitis B prophylaxis or routine prophylaxis/treatment of rubella/polio/mumps/varicella (except when VZIG is unavailable), HBIg is not covered for prevention of mother‑to‑child transmission during pregnancy, and Cytogam is contraindicated in patients with prior severe reactions to human immunoglobulins.
